Beginning November 9th 2026, we will be transitioning to a hybrid practice model that combines the personalized care of a smaller practice with the ability to continue using insurance for standard medical visits and covered services.

Under this new model, we will continue to bill your insurance for covered office visits and medical services, just as we do today. In addition, there will be an annual practice fee of:

• $300 annually for patients with commercial insurance

• $500 annually for Medicare patients

This change will allow Ashley Brown, PA-C, Dr. Gollub, and Caroline to continue spending more time with our patients, providing a more personalized approach to care, and offering greater flexibility and accessibility beyond the traditional office visit.

As many of you know, the traditional healthcare model often requires physicians to see patients every 10–15 minutes. While we have worked hard to preserve longer, more meaningful visits as our practice has grown, we have reached an important crossroads. We can continue to grow within the traditional model, which would ultimately mean less accessibility and shorter visits, or we can intentionally create a smaller, more personalized practice.

Our goal has always been to provide healthcare that feels personal, thoughtful, and accessible. We believe this hybrid model allows us to preserve what makes our practice special while giving us the time and flexibility to focus on your overall health and long-term well-being.

The annual practice fee provides additional access and personalized services, including:

• Personalized functional medicine care

• Direct messaging through the patient portal with Ashley, Caroline, or Dr. Gollub

• Same-day appointments when available

• A responsive team that knows you, your history, and your individual needs

• 10% off Opus Plasma Skin Resurfacing treatments
